In this month's webinar we explored primary care through a patient-centered and relationally coordinated lens. Farbod Hagigi of ClinicalBox, Inc. will begin our discussion with a macro look at the system-level dynamics that shape primary care and present the challenges and opportunities for patient-centeredness. Rushika Fernandopulle of Iora Health will then present a meso-level perspective of innovative approaches for redesigning care delivery and payment structures as well as helping patients navigate the primary care continuum through the use of health coaches. Finally, Leigh Simmons of Mass General Hospital will bring a micro-level perspective building on her use of decision-aids and shared decision making to jump-start and foster a collaborative relationship between patient and provider.
